 Completing the SPD CHECKLIST on-line takes about 20 - 30 minutes and costs $20.00 (USD) per SPD CHECKLIST.

DISCLAIMER: The SENSORY PROCESSING DISORDER (SPD) CHECKLIST is for your own educational purposes only and is not a professional diagnosis of any kind. A formal diagnosis of a Sensory Processing Disorder (SPD) requires the services of a licensed medical professional or therapist. For a medical diagnosis, you must consult a licensed medical professional.

  • You may complete this SPD CHECKLIST for yourself, for another family member(s) or for any other individual.

  • Once submitted, your SPD CHECKLIST will be scored immediately.  You will be provided a written report by email based on all of the responses you provided.

  • We suggest you set aside about 20 - 30 minutes to thoughtfully complete all 110 questions.  Your answers are all kept private and confidential.

  • Note On Adult Use: Although it is designed to be completed by parents for their children and is geared more toward children, it may also be used by any adults who may have sensory processing issues. If you are using this an adult, please complete all questions to the best of your ability, based on your current situation.

  • Your payment of $20.00 (USD) allows you to complete the SPD CHECKLIST, submit it for scoring and provides you with a scored written report by email.  Your paid access is for one (1) time use by one (1) person only.

The SPD CHECKLIST is scored based on the responses to each of the questions by a  "YES" or a  "NO" reply.

There are 110 questions in the SPD CHECKLIST in these categories:

  4. Social Skills Issues
  5. Learning Environment Issues
  6. Auditory: Hearing, Sound Sensitivity Issues
  7. Tactile: Touch Issues
  8. Olfactory: Smell, Food, Eating Issues
  9. Proprioceptive: Deep Pressure Issues
  10. Visual Perception Issues
  11. Fine Motor Issues
  12. Gross Motor and Postural Issues

Your scored report will be displayed after submission and is based on your replies.  You will also immediately receive a report by email will contains your answers and scores also.

 

There is a $20.00 (USD) processing fee per each
SPD CHECKLIST submitted and scored.

This small fee helps to support this website project and extensive on-line resources for Sensory Processing Disorder.

Your SPD CHECKLIST payment is non-refundable once made.
